Never tried the holster without a back pad, so I can't talk about the difference. But I do feel it provides a layer of comfort.

Unfortunately T.Rex doesn’t make a holster yet for my edc (staccato hd c4x w/ tlr7 hlx). However I put this raptor backer on a separate holster I own and it made it ridiculously comfortable and enjoyable to wear.
